TOURIST VISA（旅游证件）

• One (1) application form
  一份签证申请表格
• Original passport with at least 6 months validity
  正版护照，至少六个月有效期
• A photocopy of passport
  正式护照副本，至少六个月有效期
• Two (2) recent passport size photo (white colour background)
  两张近期拍摄的彩色护照尺寸照片(白色背景)
• A photocopy of flight itinerary/ticket
  确认的机票副本
• A photocopy of hotel voucher
  确认的酒店副本
• Applicant must sign on the application form & the signature must be same as per passport shown (Part 4)
  申请者须在申请表上签名，签名必须与护照所示的相同
• For APPLICANT UNDER 18 YEARS OLD, apart from the current documents. The following documents are required:
  为不满18周岁未成年人申请中国签证的, 除提交以上申请文件外, 还需一并提交:
1. A photocopy of applicant’s Birth Certificate
   申请者的出生证明副本
2. A photocopy of applicant’s parents/guardian passport copy or Malaysia Identification Card copy
   申请者父母/监护人的护照副本或身份证副本
3. The parents/guardian shall sign on behalf for the application form
   由申请者父母/监护人一方代签表格

BUSINESS VISA（商业证件）

• Document requested are same as above
  基本文件如上
• An original recommendation letter from a company in Malaysia
  马来西亚公司正本推荐信
• An original invitation letter to the applicant from a registered company in China
  中国合作伙伴正本公司邀请函
• SSM Letter (Form49 / Form D) *Additional Document for Director*
  马来西亚公司的注册证明副本
  (Note: For applying business multiple entries visa, applicant must have at least (2) times business type visa & entry record to China (within last 2 years record) 多次进入商业签证须有至少两(2)次商务签证和进入中国的记录 (最近的两年内)

REMARKS (注)

1. Please arrange the entire document according to the sequence. Document submitted will not be returned
   所呈上的文件恕不退还
2. All photocopies must be on A4 size paper
   所复印之文件需清晰及以A4尺寸纸为准
3. Collection of passport will be five (5) working days after appointment & submission (subject to approval)
   签证需用5天工作日，视签证批准情况而定
4. Working days refer only to days when the embassy is open and does not include weekends and holidays
   工作日仅指大使馆开放的日子，不包括周末和假日
5. The following circumstances are exempt from fingerprint collection (subject to approval)
   以下情况免除采集指纹 (视签证情况而定)
   1. The applicant is under 14 years old or over 70 years old
      申请人未满 14 岁或超过 70 岁
   2. Applicants have had their fingerprints taken at the Kuala Lumpur Visa Center within the past 5 years with the same passport
      申请人持同一本护照过去 5 年内曾在吉隆坡签证中心采集过指纹
   3. Diplomatic and official visa applicants
      外交、公务签证申请人
   4. All ten fingerprints of the applicant are missing
      申请人十指指纹均缺失

Top Urgent (2 days): + RM 400

Urgent (3 days): + RM 200

NOTES:

• PASSPORT ARE COLLECT ON BEHALF OF THE CHINA VISA SERVICE CENTER
• FOR VISA APPLICATION NEED TO ATTACH WITH FLIGHT ITINERARY (refer to requirement page)
• MULTIPLE ENTRIES VISA WILL NEED TO SUBMIT EXTRA DOC (refer to requirement page)
• APPLICATION SERVICE FEES ARE CHARGE BY THE CHINA VISA SERVICE CENTER
• THE TOTAL AMOUNT OF THE FEES TO BE PAID BY AN APPLICANT = VISA + APPLICATION SERVICE FEES
• ALL PRICE STATED ARE BASED ON RINGGIT MALAYSIA (MYR)
